Credit for Employment (CFE) Parent/Guardian Approval and Registration Form (A1) (Adobe PDF Document129 KB)
Parents/guardians must sign this form to provide consent for their son/daughter to participate in a CFE.

Credit for Employment (CFE) Employer Approval Form (A2) (Adobe PDF Document 72 KB)
This form must be signed by the CFE employer in order for the student to participate in a CFE.
