Transitioning Students with Exceptional Needs from School to Community

As students enter the high school years, they must begin to plan for their options in life after high
school. The following principles should be considered when transition planning;

  • Early coordinated planning,
  • Student, parent and support network involvement,
  • Team collaboration
  • Inclusive, community-based approaches

Transition planning should consider balancing several areas including:

  • Life-long learning
  • Employment/career development
  • Building social networks and inclusion
  • Living in the community with independence

Planning can also assist students in accessing available adult supports for when they leave school.

The Student Services Unit offers consultative support to school teams who are programming for students transitioning from school to community.
