School Bus Driver Administrative Forms

Initial School Bus Operator's Endorsement Form ( 32 KB) - Complete this form for all new school bus drivers having completed the mandatory 24 hours of school bus operator instruction. The form requires the signatures of both the school division's Transportation Supervisor AND a certified School Bus Driver Instructor.

Current School Bus Operator's Endorsement Form ( 29KB) - Complete this form for currently certified school bus drivers having completed the annual eight hours of school bus driver inservice training. This form can also be used to report school bus driver additions, deletions, or transfers. The form requires the signature of the school division's Transportation Supervisor only.

NOTE: School bus driver instruction must be provided by a person certified as a School Bus Driver Instructor. A certified School Bus Driver Instructor is considered to be a driver trainer holding a Phase III School Bus Driver Instructor's Certificate issued by the the Pupil Transportation Unit, Manitoba Education, Citizenship and Youth. School Bus Drive Consent Form ( 29 KB) - must be signed by all school bus operator certificate holders. This consent allows the Department of Driver and Vehicle Licensing to provide the Pupil Transportation Unit (PTU) with personal information regarding their Manitoba driver's license. PTU requires this information to administer the School Bus Operator's Certificate program and maintain accurate information on school bus driver license status.

Driver Qualification File Checklist ( 54 KB) - Use to ensure that the necessary forms have been completed for each school bus driver.

Driver Disclosure of Licence ( 65 KB) - An individual from the school division must meet annually with each school bus driver to have this form completed.

Annual Review of Driving Record ( 61 KB) - The individual who meets with each school bus driver completes this form annually.

Driver Record Inquiry from Driver and Vehicle Licensing ( 114 KB) - Each school bus driver must sign this form annually to authorize the school division to obtain driver records.

Driver's Certificate of Violations and Accidents ( 48 KB) - Must be completed annually by each school bus driver.

Driver's Road Test Examination ( 55 KB) - This form is used by school division examiners during their annual review of a school bus driver's fitness.

Vehicle Service Record Checklist ( 54 KB) - Use to ensure that school bus service facility staff are maintaining appropriate records.
