Municipal Numbers: Form 1 (Statement of Taxes Collected and Education Taxes Remitted) now has a line for the entry of the 3 digit municipal number. Municipalities are asked to identify themselves by both name and number.
Statement of Taxes Collected and Education Taxes Remitted:
The “Statement of Taxes Collected and Education Taxes Remitted” is available in both Excel and Adobe PDF formats. Data can be entered and saved in the Excel version. Formulas in the file calculate the correct percentages, year-to-date remittances and the net special levy based on the gross special levy and ETPC amounts. The Adobe PDF form must be printed for manual completion on paper. The Adobe PDF form requires Acrobat Reader - see link on the left for a free download.

To save a copy of these files to your computer,
- right-click the file of choice,
- Select “Save Target As...” from the pop-up menu (in Netscape, “Save Link Target As...” or Firefox, “Save Link As...”),
- Select the folder where you would like to place the file, click Save.
