Forms and Package Submission

The nomination package, including all seven required documents as listed below, must be submitted in hard copy prior to the submission deadline of December 21, 2009.

1. Award Nomination and Endorsement Form ( 1.9 MB) (completed by the Nominator)
2. Award Nomination and Endorsement Form ( 1.9 MB) (completed by the Endorser)
3. Nomination Support Form (completed by the Nominator):

• Teaching Excellence Award ( 2.2 MB)
• Outstanding New Teacher Award ( 2,2 MB)
• Team Collaboration Award ( 2.2 MB)
• Outstanding School Leader Award ( 2.2 MB)

4. Nomination Support Form (completed by the Endorser):

• Teaching Excellence Award ( 2.2 MB)
• Outstanding New Teacher Award ( 2.2 MB)
• Team Collaboration Award ( 2.2 MB)
• Outstanding School Leader Award ( 2.2 MB)

5. Nominee Consent Form ( 1.6 MB)
6. Letter of Support from the Nominee’s Supervisor (on school or school division letterhead)
7. Manitoba Teaching Certificate of the Nominee

Only complete packages including all appropriate signatures will be considered. CDs and DVDs will not be assessed.

Faxed nomination packages will not be accepted.
