Forms and Package Submission

The nomination package, including all seven required documents as listed below, must be submitted in hard copy prior to the submission deadline of December 14, 2011.

Nomination deadline extended to December 22, 2011.

1. Award Nomination and Endorsement Form to be completed by the Nominator. ( 1.45 MB)
2. Award Nomination and Endorsement Form to be completed by the Endorser. ( 1.46 MB)
3. Nomination Support Form to be completed by the Nominator:

• Teaching Excellence Award ( 3.50 MB)
• Outstanding New Teacher Award ( 3.50 MB)
• Team Collaboration Award ( 3.49 MB)
• Outstanding School Leader Award ( 3.49 MB)

4. Nomination Support Form to be completed by the Endorser:

• Teaching Excellence Award ( 3.50 MB)
• Outstanding New Teacher Award ( 3.49 MB)
• Team Collaboration Award ( 3.49 MB)
• Outstanding School Leader Award ( 3.49 MB)

5. Nomination Consent Form to be completed by the Nominee. ( 1.00 MB)
6. Letter of Support (on school or school division letterhead) from the Nominee’s Supervisor, other than the nominator or endorser.
7. A photocopy of the Nominee’s Manitoba Teaching Certificate.

Please note that the forms must be completed electronically.

Only complete packages including all appropriate signatures will be considered. CDs and DVDs will not be assessed.
