Help with Forms
To fill out the form, use the mouse to put the cursor in the first blank and click; then type the information. To get to the next text box, you can either use the mouse to move the cursor or press the tab key. Down arrows indicate choices. Click on the down arrow and scroll down. The options will appear. Click on the one that is correct or add information when the "Other" option is available.
When you have entered all the information, make a copy of your order by clicking the "Print" button on your browser, and then click on the "Submit form" button. The form will be sent and you will receive a confirmation that the form has been submitted. If you do not wish to send the form, click on the "Cancel form" button.
